On this page, you’ll find a centralized hub of all the forms related to camp! Please read each description carefully. Once your Medical Packet is completed and signed, you will then need to upload the forms to your online CampInTouch Portal.

Medical Packet: This two-page document must be completed for EACH camper, CIT, or Staff member It MUST be filled out and signed/stamped by a doctor, plus signed by a parent/guardian for those under the age of 18. In addition, you will be asked to upload a copy of your immunizations.

Health Insurance Card: You will be asked to upload an image of the front and back of your health insurance card.

Clergy Recommendation Form: All CIT, Staff, and Volunteer applicants are required to request that their pastor complete a Clergy Recommendation Form online for each participant. If you had this form completed for another program this year, then it is still valid. 

Code of Conduct: All camp participants must agree to the Code of Conduct during their online registration. It details the expected behaviors for living in a close-knit Christian camp community.

Media Consent Form: All camp participants must agree or disagree to the Media Consent Form during their online registration. It permits us to take various visual and audio recordings of the camp participants during the camp season. Names are never associated with photographs, videos, or audio. 

Online Social Networking Policy: All camp participants must agree to the Online Social Networking Policy during their online registration. It details the expected behaviors of camp participants throughout all their online presence as representatives of the camp program.
